LET ME TELL YOU ABOUT US:
We are a life insurance company which contracts medical examiners to perform insurance physicals on our clients. The examiners get paid by their employer a set fee for each exam that they perform on our behalf. There are several hundred examiner branches across the U.S. and several thousand examiners. It is very difficult for us to control the quality in which each exam is performed since these are contract workers spread all over the country. We would like to give them an extra $50.00 for each exam they perform on our behalf. The website will allow the examiner to login and complete a questionnaire stating that they met the criteria for going above and beyond their regular duties.
HERE IS WHAT WE NEED:
Website, logo and html/css files. The $1000.00 award requirements are: All images in .psd, jpg or .ai format. Include all layers. We would like to be able to edit any text on buttons, icons or menu items you use so please include a text layer. If youre using anything other than web-safe fonts please include the name of the fonts and the actual font file. The logo should have the name CheckUpChecks in it. The $500.00 award requirements are: all functional HTML/CSS files for the website layout template. Per CrowdSpring we will pay this out through our Paypal account because they do not have a category for a coded website.
You just need to post the questionnaire page which should contain the following:

OUR TARGET AUDIENCE IS:
Medical examiners in need of an incentive to go above and beyond their job description. We want the medical examiners to know we're willing to pay them extra money for their extra effort.
WE ABSOLUTELY MUST HAVE (or we don't want to see) THIS IN OUR DESIGN:
A 3 dimensional feel. Medical in nature. Ease of use. Since the only interaction with the user will be them checking off a check list we want it to look a little bit more exciting than just a check list that they complete. Youre the artist so be creative.
