(The underlying theme of this project is this is a written document representing a collaborative effort between patient and healthcare provider(s), the purpose of which is to end up treating a patient who is dying exactly as he/she wishes to be treated, whether at home, nursing home, ambulance, hospital, medical office, etc.)
