What content must be included in the website design?

There will no long winded paragraphs used anywhere in the site with the exception of the actual questions. Content will be very short and snappy (some bulleted).

Home page will essentially have the following content structure:

Name of questionnaire, 1-2 sentance on what the questionnaire is for, brief (possibly bulleted?) info on questionnaire (who is aimed for, estimate time needed, number of questions), short thank you note for taking the time to complete the questionnaire

please avoid photography. Use of iconography is welcome (large/small) especially to help balance out the content.

This is a form-heavy application, therefore please pay special attention to the layout and styling of the form. You are encouraged to be creative in creating custom styles for form elements, but please ensure that they are a) obvious and b) actually possible to create... and dont forget both on and off states, as well as how errors will be shown (message and field styles)

We are looking at releasing some small questionnaires based on an existing system, and we are looking for an initial design to use for them that provides a non-brand specific look/feel.

The style should be modern (flat, simple UI), bold, and non-brand specific. It should be created in a way that it can white-labelled versions to an extent (minor style changes, updated logo etc).

Avoid photography, and where applicable (useful), simple, informative icons should be used (simple, restricted colour palette, possibly vector?).

Would preferably like a clean web font throughout (or range of fonts), and would prefer options to be available in Typekit. Our corporate font however is Aperçu (http://www.colophon-foundry.org/fonts/apercu). It does not need to be used in this site, but can be provided if needed for any concepts.

The main focus should be on the 'form' elements (ie the questions) as we need to be able to style a range of question types including:

multiple option checkboxes/radio, drop-down based, slider-based (see example site), multiple questions per page, free-text

(this is just a selection, so it may be worth thinking about a few other variations to show how the design would work/cope).

The questionnaires are going to be relatively straight forward (page by page), and will be used for market research. Example (branded) versions can be seen at https://cx.oracle-dashboard.com and https://soa.oracle-dashboard.com (ignore the "shopping basket" and login/register areas).

The site will consist of

1) introduction (home) page
2) pre-assessment (pre qualification) page (essentially a variation of a question page)
3) question page - (range of question styles, based in similar container) - should also show a progress bar
4) thank you / results page

The site will be used across a range of devices, so design would ideally be responsive compatible (at least to allow for mobile).

A reminder on the brief. This is for a questionnaire tool, and not a for a home page for our website.

The PDF brochure uploaded by us is merely to give an idea of our existing design, but please note that this tool does not need to follow our brand. The most crucial area we are looking for is the design of the questionnaire itself.

Just a little reminder about the last sentence of the original brief, in that we are looking to have this tool usable against mobile and desktop browsers, so the design should be at least responsive to those 2 types of device.

Also, if any imagery is created, please can you allow for it being used on Retina (HiDPI) displays (such as iphone and retina macbook). ie. have a @x2 version of the images to hand
