We must first find a solution for the way we place our products visible on one place (like I briefed you this morning). The form could be smaller and Google Maps integrated. Don't you think?

Ok, here is the contact page. You didn't specify what exactly you need in this page, so I basically took the form fields from the existing page and implemented them in this new design. The right sidebar would be useful for related information, such as the contact info.
Also here is an option for the header without using the monitor screen. It does not necessarily need to be gears as a concept, but the idea is using introductory text in the left side and graphically displaying the meaning in the right side - a usually approach on many product related sites.
