Creative brief

Who are we?
We’re a web 2.0 company in the online media field and we need re-designs for 5 online question modules.

What we need?
We are looking for re-designs for 5 simple, stand alone modules, that are capable of being inserted or integrated into a webpage. They cover the 5 question types:

single line text
multi line text
drop down
radio button
check box

Since each module is simple, we have grouped them into this one project.

We are interested in seeing ONE design theme for all 5 modules. So this means that all 5 modules are part of a design theme, all looking similar and so joined together by this design theme. I have attached 2 examples of different themes to help make this clearer.

Design style?
We are open to any design style that is thrown our way. Themes could be colourful or simple, professional or fun, its really up to you! All we ask is that there is consistency across all 5 modules, with the design theme that you choose to work with.
